What’s on it

  • Four to six weeks before
  • Two to four weeks before
  • The week before, and move day
  • The first 30 days: weekly notes on eating, sleeping and mood
  • What to tell the nurse right away
  • After the first month

How to use it

Check off each step as you go.

Tell the nurse promptly about a fall, weight loss or a big change that isn’t settling. Sudden confusion or a rapid change in alertness needs urgent medical assessment, and in an emergency, call 911.

Where it comes from

This tool comes from Chapter 15, “Making the Move,” of When Mom or Dad Can No Longer Manage Alone by Mark Duda. The chapter covers making the move.

All 18 tools from the book are on the printable tools page.

The tools may be printed and copied for personal use within your own family. They are not medical, legal or financial advice.
